Training Verifications
The VCU Department of Surgery provides official verification of successful completion of training to any past graduate of a surgery training program (internship, residency, or fellowship) and to a credentialing body using our standard verification letter and/or your institutional form.

There are two steps to request your training verification. Please use the dropdown menus below to view each step:
Please complete the linked REDCap Verification of Training form so we can access the necessary information to process your request. In addition, you will need to submit a release of information form with your responses. You may use our department's Release of Information form or another release of information form from your institution.
There is a $30.00 charge for verifications of training of programs completed prior to 2024. To submit payment, please use our department storefront link.
Training verifications can take up to 30 business days to process, with longer turnaround times during onboarding and recruitment seasons. Please contact Brianna Croxton at Brianna.Croxton@vcuhealth.org if you need an expedited request.
Responses to requests will only be returned during normal business hours Monday-Friday between 9AM-5PM ET.
